NEW PATIENT CHECKLIST
First time seeing us? We'll need a few things before you arrive.

Uthrive uses JANE, an online booking software. Once you have created a profile, you can book and cancel appointments or add yourself to a practitioner's waitlist.
CREATE A JANE ACCOUNT
Clicking any of the green "Book Now/Book Online" buttons on our website will take you to the JANE booking site. From there, you can choose a service and practitioner. You will see each practitioner's schedule to select your appointment time.
Clicking the "Book With..." button under a practitioner's bio will take you directly to their JANE booking page.
COMPLETE THE INTAKE FORM(S)
Before we can treat you, we must have a completed and signed intake form. We recommend filling it out online through your JANE account, as that allows for more treatment time and saves paper. However, if you are unable to fill out the forms online, please come 10-15 minutes before your appointment to avoid delays in treatment time.
Please note: each service has its own intake form. If you are already a patient at Uthrive, and you are seeing a new practitioner, you will need to fill out their intake form.
Not sure if you filled out your forms?
Login to your Uthrive JANE account, click "My Account" and select"Intake Forms" from the left-side menu. Be sure to include your E-signature at the bottom and hit "Submit"
READ AND SIGN THE CONSENT FORMS
As Uthrive has multiple services with different practitioners, we require you to read and sign a consent form for each service you receive. For most of our services, the consent form is at the bottom of the online intake form.
We also require you to sign a consent form to direct bill your insurance provider. When you arrive for your first appointment, please bring your insurance information for us to add to your file. We will ask for you to sign the insurance consent form then.
BRING EXTENDED HEALTH CARE INFORMATION

For us to submit for you, we require your Plan/Policy/Group Number and your Member ID.
Due to the Canadian Personal Privacy Act, we are unable to access any specific information from your insurance company regarding your health plan. We recommend checking with your insurance company before coming to your appointment. Each plan is different, so it helps to know what services your plan covers. Additional information could include if they allow direct billing, and if you're required to provide a doctor's note before submitting a claim.
BRING OR SEND OVER IMAGING REPORTS
If you have received an x-ray or lab report that is related to your appointment with us, ask the clinic to fax us an imaging report. This will help your practitioner design a treatment plan before your visit.
