First time seeing us? Here are a few things we require you to do before you come.

COMPLETE THE INTAKE FORM(S)
Before we can treat you, we must have a completed and signed intake form. We recommend filling it out online through your JANE account, as that allows for more treatment time and saves paper. However, if you are unable to fill out the forms online, please come 15-20 minutes early to your appointment to avoid running into your appointment time.
Not sure if you filled out your forms? Login to your Uthrive online booking account, click "My Account" and select the "Intake Forms" option from the menu.
READ AND SIGN THE CONSENT FORMS
As Uthrive has many services, we require you to read and sign a consent form for each of the services that you receive. For most of our services, the consent form is at the bottom of the online intake form.
We also require that you sign a consent form if you have insurance that you would like us to direct bill. When you come to your appointment, we will ask for you to sign the insurance consent form then.
BRING OR SEND OVER IMAGING REPORTS
If you have gotten an x-ray or lab report that might be related to an issue you are coming in for, ask the clinic to fax us an imaging report. That way, the treating practitioner can get an idea for treatment.

BRING EXTENDED HEALTH CARE INFORMATION
At Uthrive, we do direct bill to many insurances. To see what insurance companies we can direct bill, please see the "Insurance Information" tab on our website. For us to be able to submit for you, we require your Plan/Policy/Group Number and your Member ID.
Uthrive recommends that you check with your insurance company before coming to your appointment. Each plan is different, so it is beneficial to know the services your plan covers, whether they allow direct billing and if you must submit a doctor's note before submitting a claim.
