Anatomy of the Rotator Cuff
The shoulder is a highly mobile ball-and-socket joint formed primarily by the head of the humerus and the glenoid of the shoulder blade.

Supraspinatus – assists with raising the arm and contributes to shoulder stability.
Infraspinatus – primarily helps rotate the arm outward.
Teres Minor – assists with external rotation and stabilization of the shoulder.
Subscapularis – primarily rotates the arm inward and contributes to stability.
The tendons of these muscles surround the head of the humerus and work together to help keep the ball of the shoulder centred within the socket as the arm moves.
Because the shoulder allows such a large range of motion, coordinated function of the rotator cuff, shoulder blade muscles, joints, and surrounding tissues is important for comfortable movement.
What Is a Rotator Cuff Injury?
“Rotator cuff injury” is a broad term describing problems affecting one or more of the rotator cuff muscles or tendons.
These problems exist along a spectrum and may include:
Rotator Cuff Tendinopathy
Tendinopathy refers to changes within a rotator cuff tendon that may be associated with pain and reduced tolerance to load. It often develops gradually and may be related to repetitive activity, sudden increases in training, occupational demands, or age-related tendon changes.
Rotator Cuff Strain
A strain can occur when a rotator cuff muscle or tendon is overloaded or injured. This may happen during lifting, sports, a sudden pulling movement, or another acute event.
Partial-Thickness Rotator Cuff Tear
A partial tear involves damage to part of the thickness of a rotator cuff tendon without completely disrupting the tendon.
Some partial tears are painful, while others may exist without causing significant symptoms.
Full-Thickness Rotator Cuff Tear
A full-thickness tear extends through the entire thickness of the tendon. These tears can occur after trauma or develop gradually as part of degenerative tendon changes.
Importantly, identifying a tear on imaging does not automatically mean surgery is required. Age, symptoms, strength, functional limitations, size and location of the tear, how the injury occurred, and the patient's goals all influence treatment decisions.

Tendinopathy vs. Rotator Cuff Tear: What's the Difference?
These terms can sound concerning when they appear on an ultrasound or MRI report, but they describe different types of tendon changes.
Tendinopathy generally describes changes within the tendon associated with reduced capacity to tolerate load. Symptoms commonly develop gradually and may fluctuate depending on activity.
A rotator cuff tear means some of the tendon fibres have been disrupted. Tears may be partial or full thickness and can develop from trauma or gradually over time.
Rotator cuff tendinopathy and tears can occur in any of the rotator cuff tendons, but most commonly affect the supraspinatus tendon as shown in the image above.
One important point is that imaging findings and symptoms do not always match perfectly. Rotator cuff tears and other structural changes become increasingly common with age and may be present in people who have little or no shoulder pain.
For this reason, imaging results should usually be interpreted alongside your symptoms, strength, function, physical examination, age, activity level, and how the injury occurred.
Common Causes & Risk Factors
Rotator cuff problems do not always have one identifiable cause. They may develop gradually or occur following a specific injury.

Shoulder and Upper Arm Pain
Pain is often felt around the front or outside of the shoulder and may extend down the upper arm.
Pain With Lifting the Arm
Reaching overhead or lifting the arm away from the body may reproduce symptoms.
Pain With Reaching Behind the Back
Putting on a jacket, fastening a bra, reaching into a back pocket, or other movements behind the body may be uncomfortable.
Shoulder Weakness
The arm may feel weaker when lifting, reaching, carrying, or rotating the shoulder.
Significant weakness following an acute injury deserves further assessment.
Night Pain
Some people have difficulty sleeping on the affected shoulder or experience aching at night.
Pain During Exercise
Bench pressing, overhead pressing, push-ups, pull-ups, throwing, swimming, and other upper-body activities may become uncomfortable.
How Is a Rotator Cuff Injury Diagnosed?
Diagnosis typically begins with a detailed history and physical examination.
An assessment may include evaluating:

Is Imaging Necessary?
Not always.
Many uncomplicated cases of shoulder pain can initially be managed without imaging.
Ultrasound or MRI may be considered when there has been significant trauma, substantial weakness, suspected full-thickness tearing, persistent symptoms despite appropriate conservative treatment, or when imaging would influence treatment decisions.
X-rays may also be useful when arthritis, fracture, calcification, or other bony conditions are suspected.
What Helps Rotator Cuff Injuries?
Treatment depends on the type and severity of the injury, but conservative management is often appropriate.
Activity Modification
Temporarily reducing or modifying activities that repeatedly aggravate the shoulder can allow symptoms to settle while maintaining as much normal activity as possible.
Progressive Exercise
Exercise is an important component of rehabilitation.
Treatment may gradually target:

Can I Still Exercise With a Rotator Cuff Injury?

You usually do not need to stop exercising completely because of rotator cuff pain. Instead, exercises may need to be temporarily modified based on symptoms and the severity of the injury.

When Should I Seek an Assessment?

Yes. Pain around the shoulder does not always originate from the rotator cuff.
Other possible sources include:

Cervical Spine: Problems involving the neck can refer pain into the shoulder and upper arm. Nerve irritation may also cause pain, numbness, tingling, or weakness extending into the arm.

AC Joint: The acromioclavicular joint at the top of the shoulder can become painful following trauma or repetitive loading.

Frozen Shoulder: Adhesive capsulitis typically causes progressive pain accompanied by substantial restriction in shoulder movement.

Glenohumeral joint arthritis, instability, labral injuries, and other problems within the shoulder joint can produce symptoms similar to rotator cuff pain.

Biceps Tendon: The long head of the biceps tendon can contribute to pain near the front of the shoulder.
